| Necrotizing scleritis is a rare and sinister complication of pterygium excision surgery using anti-metabolites. Controlling the inflammation and managing such cases with impending perforation is important for visual prognosis, but the means to achieve this has not been described at length. In pterygium tissue, both the message and protein expression of insulin-like growth factor binding protein-3 (IGFBP3) are decreased compared with normal, uninvolved conjunctiva. This suggests that there is loss of the control mechanism of cell proliferation, which may explain the continued growth of the pterygium. Using this property, we successfully treated a patient with necrotizing scleritis using his own pterygium tissue as a graft. To date, there has been no reported case managed as such. | |
